In rural Northwest Florida, the "hidden homeless" population isn't always visible on downtown sidewalks. Families, veterans, and individuals experiencing chronic displacement are often tucked away in remote wooded encampments, vehicle sites, and isolated rural corridors.
That is why we created WE CARE: a hybrid mobile and digital street outreach lifeline designed to meet our community's most vulnerable neighbors exactly where they are.
🌟 What We Do: Real Connection, Real Impact
The WE CARE program operates seamlessly across Bay, Jackson, Gulf, Calhoun, Washington, and Holmes Counties through two powerful components:
1. Consistent In-Person Presence
Trust is built on consistency. On a systematic weekly rotation, our dedicated team of specialized care coordinators and volunteers deploys directly into rural sectors, local parks, and high-density resource areas.
Essential Lifelines: We provide immediate crisis intervention and basic survival resources.
The Gift of Connection: To break down isolation, we distribute activated cell phones (complete with 60 days of service) to individuals in need, ensuring they have a secure way to stay connected to case managers and regional service providers.
A Gateway to Stability: As an official access point for the Continuum of Care’s Coordinated Entry System, we complete on-site housing assessments to fast-track individuals toward permanent, stable housing.
2. 24/7 Virtual Outreach & Support
We leverage digital innovation to shatter the barriers of fear, skepticism, and geographic isolation.
Targeted Outreach: Through a robust, trauma-informed digital ad campaign, we actively reach out online to provide immediate, accessible entry points for support.
Accessible Assistance: Our website features a secure, user-friendly, and completely confidential Assistance Request Form accessible 24/7, making it easier than ever for individuals in crisis to raise their hand for help.
🤝 Special Populations We Serve
Our program, partnerships, and referral networks are specifically structured to support and empower individuals navigating high-acuity crises, including:
Survivors of domestic violence, stalking, and human trafficking
Individuals living with physical or mental health disabilities
Families with children and pregnant individuals
Youth and young adults (ages 18–24)
Veterans and individuals experiencing chronic unsheltered homelessness
🌿 Moving Beyond the Street: The Village HealthCARE
Connecting on the street is just the first step. Individuals encountered through our WE CARE program are warmly invited to transition into our Village HealthCARE initiative. This comprehensive program equips participants with critical, life-changing skills, including:
Hands-on, in-person food preparation demonstrations hosted at transitional housing partner sites.
Chronic illness management resources (such as diabetes and hypertension education).
Essential financial literacy, budgeting guidance, and whole-health educational platforms accessible around the clock.
